RAID Log Template

• Overview sheet: project details, a guide to each sheet, how to use the log, and a 5 × 5 risk scoring matrix.
• Risks: likelihood and impact scored 1 to 5, with the risk score and rating calculated automatically. It also records the mitigation, contingency plan, owner, target date and review date.
• Assumptions: the reason behind each one, the impact if it proves wrong, the owner and the review date.
• Issues: priority, who raised it, the owner, the resolution action and dates.
• Dependencies: what you depend on, from whom, the impact if it isn't met, and the date it's needed by.
• RAG dashboard: an automatic summary of open items across all four logs, with an overall project status.

1. 1.Fill in the project details on the Overview sheet.
2. 2.Add each item to the right sheet, with a unique reference (R001, A001, I001, D001).
3. 3.Give every item a named owner and a date.
4. 4.Review the log at every project meeting and update the "last reviewed" date.
5. 5.Close items rather than deleting them, so you keep the history.
